清光绪元年（1875年）置，属台湾府1。治所即今台湾省屏东县东南恒春镇2。光绪十三年（1887年），台湾府改为台南府，县属之3。光绪二十一年（1895年），清政府被迫签订中日《马关条约》，将台湾省割让于日本4。

1，《清实录》52·德宗实录1·卷3：光绪元年正月庚戌，于车城南猴洞地方建设县治，名曰恒春。（北京，中华书局，1987年，109页）。
1965年《台湾省通志稿》卷首上·疆域·第四章：光绪元年，钦差大臣沈葆桢于南部番界琅峤地方，筑城设官名曰恒春县，属台湾府，县治猴洞，在车城南十五里（台湾，成文出版社《中国方志丛书》台湾地区第64号，1册，73页）。
